What "added" means
The date is when CMS issued the NPI, which is the only timing the registry records. For an organisation it is a weaker signal: a practice often registers months before it opens, occasionally after, and a practice that rebrands or is acquired may register a new NPI without anything new opening.
Addresses are what the provider filed with CMS. They are frequently a billing office rather than where anyone sees patients, and the registry has no retirement field — some records belong to people who have stopped practising.
