How this was built
Licence states come from the taxonomy records on each NPI: a provider lists a licence number and state for each taxonomy they register. That is the only multi-state signal the registry carries.
It is an undercount, and probably a severe one. Most clinicians register a single taxonomy with a single licence even when they hold several, so the people at the top of this table are those who happened to enter more, not necessarily those licensed in the most places.
